Dignity. Access. Health. Belonging.
Grace Mobile Community Health
Taking care to the community… and the community to care.
Healthcare should not begin only when someone is able to reach a hospital.
Meet people where they live, work, gather, worship, and seek support.
Help people reach trusted healthcare facilities and return safely home.
Connect community members with appropriate clinical and support services.
Close the loop by determining whether needed care was actually received.
GRACE MOBILE COMMUNITY HEALTH
GRACE MOBILE COMMUNITY HEALTH is the community outreach, navigation, transportation, and mobile-care initiative of THE SHODIPO~ASIMOLOWO FOUNDATION.
Healthcare should not begin only when someone is able to reach a hospital.
For many people, the greatest barriers come before the appointment ever happens… transportation, cost, fear, stigma, unstable housing, lack of information, childcare responsibilities, difficulty navigating the healthcare system, or simply having no one to help connect the pieces.
GRACE MOBILE COMMUNITY HEALTH exists to close that gap.
We meet people where they are… identify what they need… connect them with appropriate care… help them get there… and follow through afterward.
Our model begins with people, not buildings.
Community outreach teams work directly within underserved communities to identify individuals and families who may otherwise remain disconnected from healthcare and essential services.
Depending upon community needs, support may include:
Community Outreach
Meeting people where they live, work, gather, worship, and seek support.
Transportation
Helping individuals reach trusted healthcare facilities and return safely to their communities.
Health Navigation
Helping people understand where to go, what services they need, and how to move through the healthcare system.
Linkage To Care
Connecting individuals with appropriate clinical services, testing, treatment, maternal care, preventive care, mental wellness support, and community resources.
Follow-Up
Because a referral alone is not enough. Our goal is to help close the loop and determine whether care was actually received.
Reaching people who are too often left behind.
GRACE MOBILE COMMUNITY HEALTH is designed to reach people who are too often left behind, including vulnerable and underserved WOMEN, children, families, older adults, young mothers, people experiencing homelessness or housing instability, individuals living with chronic illness, and communities facing barriers to healthcare access.
The initiative can also support targeted outreach for individuals requiring HIV and infectious-disease services, maternal and reproductive healthcare, preventive screenings, nutrition support, mental wellness services, medication follow-up, and other essential community health needs.
The program is intentionally flexible.
Services are shaped around the real barriers, priorities, and health needs identified within each community.
The community tells us what the community needs.
GRACE MOBILE COMMUNITY HEALTH is being developed in stages.
Build The Foundation
We begin with the infrastructure that matters most… trusted people, community relationships, transportation, navigation, healthcare partnerships, and follow-through.
Bring Services Closer
As the program grows, community health days and mobile clinical services can bring selected screenings, education, preventive services, and healthcare professionals directly into communities.
Expand Mobile Care
Ultimately, dedicated mobile health units can expand that reach even further… taking healthcare beyond traditional walls and into places where access remains limited.
This approach allows THE SHODIPO~ASIMOLOWO FOUNDATION to build services around demonstrated community needs rather than assumptions.
A service program and a listening system.
GRACE MOBILE COMMUNITY HEALTH is not only a service program.
It is also a listening system.
With appropriate privacy protections, the program can help identify recurring community needs, transportation barriers, healthcare access gaps, referral patterns, geographic areas of greatest need, and services that should ultimately be brought closer to the people.
That information can guide future Foundation clinics, community programs, partnerships, funding priorities, and permanent healthcare infrastructure.
We listen first… then we build.
Care moves with people from first contact through follow-through.
Outreach
Meet people where they are.
Transportation
No one left behind because they could not get there.
Linkage To Care
From screening to treatment… connecting the pieces.
Follow-Up
Care does not end when the appointment does.
A community health system should be able to move.
We envision communities in which geography, poverty, transportation, instability, or social circumstances no longer determine whether someone receives care.
- It should reach the WOMAN who cannot leave HER children.
- It should reach the elderly man who has no transportation.
- It should reach the young person afraid to ask for help.
- It should reach the family that does not know where to begin.
- And when necessary… it should bring them safely to the care they need.
